Yes! Brixham have been successful in securing funding from Torbay Council and the NHS to pilot a Home From Hospital scheme in Brixham. The service will work by recruiting neighbourhood volunteers who will help to support their neighbours so that they are able to leave hospital as soon as they are better.

Often people must spend unnecessary time in hospital due to not having the support at home that would enable them to leave safely, this can cause people to have to stay away from their home for longer leading to poor health outcomes. Our aim is to offer a service that can improve hospital discharge and can help build connections between neighbours in the process too.

The scheme is coordinated by our Home From Hospital Project Team, to contact them please call 01803 851414 or email info@yesbrixham.org.uk

We accept referrals for this service from Social Workers, Wellbeing Coordinators and directly from local hospitals. In order to refer into the scheme you can contact us directly or complete the following referral form
